Telehealth Informed Consent

This document explains the nature of telehealth services, your rights as a patient, and what to expect from your care experience with Premier Element Group.

What Is Telehealth?

Telehealth refers to the delivery of healthcare services using electronic communications and digital technology, allowing patients and licensed healthcare providers to interact remotely without an in-person office visit. Through Premier Element Group's platform, powered by Bask Health, you are able to complete a detailed health assessment, communicate with a licensed independent provider, and receive a prescription — all from the convenience of your home.

Telehealth services may include, but are not limited to: online health assessments, asynchronous review of patient-submitted health information by a licensed provider, prescription issuance, and ongoing care coordination and dosing support.

Your Care Team

All clinical services are delivered by licensed independent healthcare providers affiliated with Arora Health & Aesthetics, LLC. These are independent licensed physicians and clinicians who evaluate your health assessment, make all clinical determinations, and — where appropriate — issue a valid patient-specific prescription.

Premier Element Group LLC is a management services organization ("MSO") and does not practice medicine. The licensed providers of Arora Health & Aesthetics are solely responsible for all clinical decisions, including whether to prescribe treatment and at what dosage.

Compounded Medications

Prescriptions issued through Premier Element Group are fulfilled by VialsRX, a state-licensed compounding pharmacy operating under Section 503A (6220 Westpark Dr, Houston, TX 77057 · (713) 497-5590).

Privacy & Confidentiality

Your health information is protected under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"). All health data submitted through our platform is handled within a HIPAA-compliant infrastructure maintained by Bask Health under a signed Business Associate Agreement ("BAA") with Premier Element Group LLC.

We do not sell your personal information or Protected Health Information ("PHI"). Your information is used solely for the purpose of providing and coordinating your care and as otherwise described in our Privacy Policy and HIPAA Notice of Privacy Practices.

Telehealth communications conducted through our platform are transmitted using secure, encrypted technology. No portion of your consultation will be recorded without your prior knowledge and consent.

Your Patient Rights

As a patient, you have the right to:
  • Receive clear information about your diagnosis, proposed treatment, and available alternatives before consenting to care
  • Ask questions about your treatment plan and receive honest, complete answers from your provider
  • Refuse treatment at any time without penalty or loss of dignity
  • Request a copy of your medical records in accordance with applicable law
  • Withdraw your consent to telehealth services at any time by notifying our support team
  • Seek a second opinion from another licensed healthcare provider
  • Be treated with respect, dignity, and without discrimination
  • Have your protected health information kept private and secure in accordance with HIPAA

Section 09

Emergencies & Adverse Events

⚠ Important: Telehealth Is Not for Emergencies

If you are experiencing a medical emergency, chest pain, difficulty breathing, severe allergic reaction, or any other life-threatening condition, call 911 or go to the nearest emergency room immediately. Do not use this platform to seek emergency care.

If you experience an adverse reaction to a medication prescribed through your program, discontinue use and contact your assigned provider or our support team immediately. For any adverse event that requires urgent attention, seek in-person medical care first and notify our team as soon as possible thereafter.

Section 10

Withdrawal of Consent

You may withdraw your consent to receive telehealth services at any time by notifying Premier Element Group in writing at support@premierelementgroup.com. Withdrawal of consent does not retroactively affect the validity of any prescriptions already issued or any charges already processed in accordance with our Refund & Cancellation Policy.

Your provider may also withdraw from your care with reasonable notice if they determine that telehealth is no longer appropriate for your condition. In such cases, they will, where possible, provide a referral to appropriate in-person care.

Governing Law

This Telehealth Informed Consent is governed by the laws of the State of Florida and applicable federal telehealth regulations. By proceeding with enrollment, you confirm that you have read, understood, and consent to the terms described in this document.
