Technical Data: IP address, browser type, and pages visited, collected automatically.
Our telehealth platform is powered by Bask Health, a HIPAA-compliant telehealth technology provider. Patient data including PHI is processed and stored within the Bask Health platform infrastructure. Bask Health operates under a signed Business Associate Agreement (BAA) with Premier Element Group LLC.
We do not sell your personal information. We may share with: Independent licensed healthcare providers involved in your care; Licensed 503A compounding pharmacy partners to fulfill prescriptions; Bask Health platform for telehealth technology services; Other service providers bound by confidentiality agreements and BAAs; Legal authorities when required by law.
We implement industry-standard security measures including SSL/TLS encryption and access controls. No method of internet transmission is 100% secure.

These Terms govern your use of Premier Element Group LLC services. By using our site or services, you agree to these Terms.
You must be at least 18 years of age and a U.S. resident in a state where our services are available. A valid government-issued ID may be required as part of the clinical intake process.
Premier Element Group LLC operates as a Management Services Organization (MSO) providing administrative and technology services to support independent licensed healthcare providers. We do not practice medicine and do not employ healthcare providers. All clinical services are provided by independent licensed physicians through our technology platform, powered by Bask Health.
Premier Element Group operates as an MSO. Clinical care is provided by independent licensed providers through the Bask Health telehealth platform. Premier Element Group does not employ or control clinical decision-making by any provider.
Compounded medications offered through Premier Element Group — including compounded tirzepatide and other compounded formulations — are not FDA-approved and have not been evaluated by the FDA for safety, efficacy, or quality. They are prepared by licensed 503A compounding pharmacies pursuant to valid patient-specific prescriptions. Compounded tirzepatide is not the same as FDA-approved Zepbound® or Mounjaro®, and is not a generic version of these medications.
Prescriptions are only issued after evaluation by a licensed healthcare provider who determines clinical appropriateness. Prescription approval is not guaranteed. Not all patients will qualify for treatment. All clinical decisions are made solely at the discretion of the independent licensed provider.
Programs are billed monthly. Current programs include: Weight Loss ($239/month), Longevity & Wellness ($129/month), Hair & Skin Care ($84/month), and Men & Women's Wellness ($149/month, when available). Pricing is subject to change with notice. We accept major credit cards and HSA/FSA cards. No insurance billing.
Hair & Skin Care program — including compounded Minoxidil, Finasteride, Tretinoin, Fluocinolone, Latanoprost, and GHK-Cu formulations — is available as a standalone or add-on program. Finasteride is contraindicated for women who are pregnant or may become pregnant.
You may cancel at any time before your next billing date by emailing support@premierelementgroup.com. No cancellation fees apply. If your licensed provider determines the requested therapy is not clinically appropriate following review of your intake, any applicable refundable consultation fee will be handled according to the program terms. Once medication has been shipped, all medication sales are final. Shipped medications are non-refundable.
These Terms are governed by Florida law. Disputes shall be resolved in Miami-Dade County courts.

This Consent explains the nature of telehealth services provided by Premier Element Group.
Telehealth uses electronic communications to enable licensed healthcare providers to evaluate and treat patients remotely. Premier Element Group's platform supports both asynchronous consultations (intake form reviewed by provider without a live visit) and synchronous video consultations.
By using our services, you acknowledge that compounded medications prescribed through Premier Element Group — including compounded tirzepatide, NAD+, Glutathione, Sermorelin, Minoxidil, Finasteride, Tretinoin, Fluocinolone, Latanoprost, and GHK-Cu formulations — are not FDA-approved and have not been evaluated by the FDA for safety, efficacy, or quality. You consent to receive compounded medications prepared by licensed 503A compounding pharmacies pursuant to a valid prescription.
Technology failures may interrupt consultations. A provider may determine in-person care is necessary. The absence of a physical examination may limit clinical assessment. All prescriptions are issued solely at the clinical discretion of your independent licensed provider.
You acknowledge you have read this Consent, understand the nature of compounded medications, and agree to receive telehealth services under these terms.
